I. The authorizing party confirms they hold lawful authority or guardianship over the monitored individual with respect to the monitoring activities described herein.

II. The monitored individual has been informed of, and has consented to, the nature, scope, and purpose of monitoring as outlined in this agreement.

III. All data collected through monitoring activities shall be used solely for the stated purpose and handled in accordance with applicable privacy laws and regulations.

IV. This agreement remains in effect until formally revoked in writing by either party, or until the relationship giving rise to the monitoring authority ceases.
